Health Forms
In an effort to better meet the needs of individuals turning in state required health forms, MTHS is offering four options. The preferred method is electronic submission via a secure email directly to the school nurse at mthsmedical@mths.us. This process can be done by scanning or taking a picture of the documents on a smartphone and then emailing the forms to the email address listed above. Forms may also be faxed to the school nurse’s dedicated fax line at (309)-367-4251, dropped off at the school office Monday – Thursday (8:00 am - 12:30 pm) or mailed to the school marked Attention “Nurse Doty”. ALL pages must display the student’s full name and date of birth in order to be valid. To access required forms, please use the link below.
The forms normally collected by Health Services each year, along with comments and links to the forms, are listed below. Physicals and immunization records are due by the first day of school.
Free & Reduced Meals
MTHS participates in the National School Lunch Program. This program allows students to receive free or reduced lunches each school day. Families need to complete the income eligibility application to enroll in this program.
This application not only provides free or reduced meals, but can also qualify families for registration fee reductions and other school expenses. To apply for reduced or waived fees, please complete the application. Submit the completed application to Mrs. Nancy Simpson in the Superintendent's Office.
Parking Permits & Vehicle Registration
For Juniors and Seniors wishing to purchase a parking permit for the west parking lot, permits can be requested electronically via Skyward Student Access. Vehicle registration is also submitted electronically.
In order to be eligible to request and purchase a parking pass the following is required:
All fees must be paid from previous year(s). This includes food service balances, as well as any fees in arrears.
On-line registration must be completed by both parent/guardian & student. This process is completed annually.
Permits cost $50. The parking fee will be dropped into Skyward (after the student has been contacted by Mrs. Foster that a pass is available) to allow families to pay electronically, however cash and checks will also be accepted in the Main Office.
